The ºÚÁϲ»´òìÈ CORE Peer Navigator Project is seeking a qualified individual to serve as the Community Outreach Specialist. This position is vital to the ongoing success of the ºÚÁϲ»´òìÈ CORE Peer Navigator Project.
The ºÚÁϲ»´òìÈ CORE Peer Navigator Project is an innovative OMH funded program that is designed to assist HARP enrolled individuals by connecting them with a highly trained Peer Navigator who will accompany them throughout the process of awareness, interest, referral and self-determined enrollment and involvement in OMH’s Community Oriented Recovery and Empowerment (CORE) services. CORE services are person-centered, recovery-oriented mobile behavioral health supports that include:

Community Psychiatric Support and Treatment (CPST): multi-component service of goal-directed and solution-focused supports that includes therapeutic interventions

Family Support and Training (FST): offers instruction, emotional support, and skill building for an individual’s family of choice

Psychosocial Rehabilitation (PSR): assists individuals in improving their functional abilities to the greatest degree possible in settings where they live, work, learn, and socialize

Empowerment Services (Peer Support): non-clinical, peer-delivered services with focus on rehabilitation, recovery, and resilience.
The Community Outreach Specialist’s primary job duties are centered around reaching out and forming relationships with a broad array of community behavioral health, medical and social service agencies, and stakeholders from across ºÚÁϲ»´òìÈ State to encourage referrals and to support participation in local CORE services.
