30 Minutes on Monday: Care Transitions Network Overview

ϲ Note: Join the National Council for Behavioral Health for a 30 minute overview of their new initiative, “Care Transitions Network for people with Serious Mental Illness”. Learn how the Care Transitions Network will assist organizations and providers in ϲ State to reduce all-cause re-hospitalization rates for people with serious mental health challenges. The Care Transitions Network is made possible by a grant given to the National Council for Behavioral Health by the Centers for Medicare and Medicaid Services, in a partnership between the National Council, Montefiore Medical Center, Northwell Health (formerly North Shore-LIJ Health System), the ϲ State Office of Mental Health and Netsmart Technologies.

The National Council for Behavioral Health and its partners are pleased to invite you to a 30-minute overview of the , a four-year initiative to support providers across ϲ State.

Care Transitions Network Informational Webinar

Learn how the Care Transitions Network is helping organizations and providers in ϲ State reduce all-cause re-hospitalization rates for people with serious mental illness.

This webinar will illuminate how the Care Transitions Network will help enrolled organizations and providers transition to managed care and value-based care, while receiving individualized support and resources including:

  • Care transitions support to facilitate and ensure engagement with outpatient practices for patients after discharge from psychiatric hospitalizations;
  • Expert faculty to offer real-time clinical consultation, targeted coaching and training on implementation of evidence-based practices and the transition to value-based payment arrangements;
  • A web-based platform to enable your organization to track progress for continuous quality improvement (for organizations that want it, free licensure and implementation support is available to adopt Netsmart’s Care Manager Solution to be used as part of this project) and
  • Up to $1,000 per enrolled eligible professional over the course of this initiative.
